Mason Public Schools Transportation Request Form

Dear Parent(s) and Guardian(s)

To ensure that all our students are being safely transported, we ask that you complete a transportation request form if your student(s) will require busing by 11:59pm on August 1, 2022. Families will receive their bus route information before the first day of school. If this form is not submitted by this date/time, we cannot guarantee your student(s) will be included in a bus route for the first few days of school and self transport will be needed by families.
